Appointments

Appointment times are specially designated for each medical need, such as: new patient visit; follow-up visit; diagnostic testing; teaching (pump or injections); testing result and treatment recommendation.


To MAKE an appointment:

To make an appointment, call our main line during business hours: (614) 839-3040, or complete online form below. When completing online form, allow for 48 hours before expecting to hear back from us on when your appointment is set. When you call, please have your insurance information (if any) and referral number (if you are a member of an HMO or PPO) available. New patients must obtain a referral for an initial office visit. Most insurance companies require us to verify your coverage, deductible, and/or co- insurance amounts before treatment. This process is completed prior to your appointment using the carrier information you provide at this time. 

 

To CHANGE an appointment:                               

When you are given an appointment with us, that specific time is reserved exclusively for you. Absent an unavoidable and unpredictable medical emergency, we make considerable efforts to see all patients within 30 minutes of the appointed time, and we do not double-book or over-book patients as some other medical practices do. Consequently, in courteous fairness to everyone, we ask that you give us several days' notice in the event you cannot keep any appointment with us. When changing an appointment, be aware that there may be a month or more delay before we can re-schedule your visit. To CANCEL an appointment:

Please call us with cancellations 24 hours prior to your scheduled appointment time. No show appointments result in a $75 missed appointment fee. In the event that your medical insurance does not cover this charge, you will be responsible for payment. More than three sequential cancellations or "no show" appointments may result in the termination of our medical relationship. You can MAKE, CANCEL, or RESCHEDULE appointments by completing the following information. We will call or e-mail you back to confirm your appointment. Note that fields with astericks (*) are required. We will do our best to accommodate your appointment request and will give a confirmation of receipt within three business days of receiving your request during Monday through Thursday, 8:00 a.m. to 5:00 p.m., Friday, 8:00 a.m. to 4:00 p.m. business hours.

EVENING or EMERGENCY appointments:

For urgent and same day appointments, please telephone the office at (614) 839-3040. Appointments scheduled as “emergencies” (an urgent medical need) either during normal working hours, after week-day hours, or on weekends will be subject to an additional charge. In the event that your medical insurance does not cover this charge, you will be responsible for payment.
